IPAC Policies and Laundry Workflows in Senior Living and LTC

Infection Prevention and Control (IPAC) is not just about hand hygiene and surface disinfection. In senior living, assisted living, and long-term care (LTC), linen and laundry workflows are a major vector for pathogen movement. A single breach in sorting, transport, or handling can turn clean linens into a cross-contamination risk.

This article explains how Ontario care homes currently manage laundry under IPAC guidance, where the common failure points are, and how a robotic laundry assistant can be designed into the workflow without weakening compliance.

Why laundry is an IPAC issue

Residents in LTC and assisted living are often immunocompromised, have open wounds, or live with indwelling devices. Linens become soiled with sweat, skin flora, blood, feces, urine, and respiratory droplets. If those linens are shaken, sorted incorrectly, or transported through clean corridors without a covered cart, organisms can spread.

Provincial guidance — including Ontario's Best Practices for Infection Prevention and Control in Long-Term Care and the companion guidance on linen handling — expects operators to treat soiled and clean linen as distinct material flows. The goal is simple: keep soiled linen contained, clean linen protected, and staff hands away from unnecessary exposure.

The standard laundry workflow

Most Ontario LTC laundry rooms follow a seven-stage workflow. Each stage has its own IPAC requirements:

  1. Collection: Soiled linens are placed in leak-proof bags or covered hampers at the point of use. Staff avoid shaking or sorting linens in resident rooms.
  2. Transport: Soiled linen carts are covered, labelled, and routed separately from clean linen carts. They should never share an elevator or corridor at the same time as clean linens if avoidable.
  3. Sorting: At the laundry, soiled linens are sorted by type, temperature tolerance, and soil level. This is one of the highest-exposure tasks because it happens before washing.
  4. Washing and drying: Thermal or chemical disinfection is verified through washer settings, detergent chemistry, and drying temperature. Healthcare linens typically require hot water cycles.
  5. Folding and finishing: Clean, dry linens are folded and staged for distribution. This stage is lower risk but still requires clean hands and a clean surface.
  6. Storage: Clean linens are stored in a dedicated, dry, enclosed area away from soiled linen and cleaning chemicals.
  7. Distribution: Clean linens are delivered to units or resident rooms using covered carts or closed bags.

Common IPAC failure points

Audits in Ontario LTC homes repeatedly find the same issues. Most are operational, not clinical:

  • Cross-traffic: Clean and soiled carts passing each other in the same corridor or elevator.
  • Overfilled hampers: Bags tied incorrectly or hampers left uncovered in hallways.
  • Manual sorting without PPE: Staff sorting soiled linen without gloves or gowns, especially during short-staffed shifts.
  • Shared surfaces: Folding tables used for both clean and soiled items without intermediate cleaning.
  • Inconsistent hand hygiene: Gloves worn but hands not cleaned after removal.

These failures are rarely intentional. They happen when laundry aides are stretched across too many tasks, when the physical layout forces clean and soiled paths to cross, and when repetitive work leads to normalization of deviation.

Where automation fits — and where it does not

A robotic laundry assistant is not a replacement for an IPAC program. It is a tool that can make the program more consistent by removing variability from specific, repeatable tasks.

Good candidates for automation include:

  • Folding clean, dry linens: A robot can fold towels, sheets, and clothing to a consistent standard on a dedicated clean surface, reducing the time staff spend in the laundry room.
  • Sorting clean linen by destination: After washing and drying, linens can be sorted by unit, room, or type without human contact.
  • Internal transport: A wheeled base can move clean linen carts along a predefined route, keeping the flow predictable and separate from soiled-linen paths.

Tasks that should remain under human control and clinical oversight include:

  • Handling soiled linen before washing: Sorting soiled items, managing isolation linens, and deciding wash parameters remain human responsibilities.
  • Verification and auditing: Wash temperatures, chemical dosing, and infection control audits must still be performed and documented by staff.
  • Resident-facing delivery: Placing linens in resident rooms or on beds often requires judgment, privacy, and interpersonal care.

Designing a robot into an IPAC-compliant workflow

For automation to strengthen rather than weaken infection control, the robot must be treated as part of the clean-linen flow, not a general-purpose helper. Key design principles include:

  • Dedicated clean zone: The robot works only with linens that have already been washed, dried, and thermally disinfected.
  • Surface compatibility: All touch surfaces that contact linen must be non-porous and cleanable with facility-approved disinfectants.
  • Clear separation of flows: The robot's route from folding to storage or staging should not overlap with soiled-linen transport.
  • Traceability: Each load folded or moved by the robot is logged with a timestamp, load ID, and destination for audit purposes.
  • Maintenance hygiene: Scheduled cleaning of the robot's end effectors, tray, and wheels is built into the operating protocol.

Practical IPAC checklist for laundry automation
